Okay now why Otaru and Keiko would have problems as a couple. Firstly and most importantly--OTARU DOES NOT KNOW THAT HIS BEST FRIEND IS REALLY A GIRL! I'm not saying that his best friend shouldn't be a girl, I'm just saying that this "best friend" is decieving him. If you didn't already understand, Otaru's best friend is not Amuro Keiko, it's Fushige Kei. So all this time so far, Otaru thinks he found his very best friend in the world, someone he thinks he can trust. Ouch to him. Also, I do admit that I think Keiko has a slight crush on Otaru and maybe Otaru might have a crush on his best friend weird as it is. But then don't you see, that Keiko's making Otaru think that he's gay? I say that she'd better tell him the truth sometime soon...she's making him question his lifestyle and also, what will happen when he finds out that he's been decieved the whole time? Exactly. It would kinda be hard to forget about that whole situation. Nice friendship going on there.
